Submit a request for verification online via the Veridoc Website . Enter all of the required information.
Note: All states accept VeriDoc verifications. The Iowa Board of Medicine will not complete other state medical board license verification forms. The standard verification letter generated through VeriDoc provides all of the information required by state medical boards.
